APP Supports Coordination Organization was founded in 2022 with a focus on providing quality services to the individuals and families we serve. APP’s leadership brings years of experience supporting individuals with Autism and Intellectual Disabilities and providing Supports Coordination for those in need of services and currently participating in P/FDS, Community Living, and Consolidated Waivers in various counties throughout PA.
Our mission is to provide individuals with the supports, opportunities, and connections they need to find their own path in life.
At APP we believe that individuals have the right to live their life to the fullest, be as independent as possible, and to make their own choices. APP strives to educate individuals and families, so that those served can spend each day living a life they love, a life which makes them happy, and a life which makes them feel fulfilled.

Meet Our Founder
Brittany McCord
Brittany graduated from Indiana University of Pennsylvania with a bachelor’s degree in Social Studies Education and a minor in Educational Psychology. She began her career teaching in both the public and private sectors, educating, and working with adolescents and at-risk youth. After a couple years of teaching, Brittany stumbled into the field of Supports Coordination in 2018 and has been serving families ever since. She has supported individuals and families receiving services through Consolidated, Community Living, and P/FDS Waivers. Additionally, Brittany has supported non-waiver participants in navigating grants, community resources, and other disability services.

What is Supports Coordination?
Supports Coordination involves the locating, coordinating, and monitoring of services and supports. The Supports Coordinator (SC) assists participants in obtaining and coordinating needed waiver and other State plan services, as well as housing, medical, social, vocational, and other community services, within the waiver programs and other community resources. The service includes both the development of an Individual Support Plan (ISP) and ongoing supports coordination.

We heavily support family involvement and a collaborative team approach. Two heads are better than one, right?
Caseload sizes do not exceed 30 individuals to ensure the focus is on the quality of service we provide to our individuals.
We respond to all communication within 24-48 hours during typical business hours.
Supervision occurs on a monthly basis for all Supports Coordinators and bi-weekly during big life changes to ensure all participants needs are met.
We believe in keeping our organization small, so we can best meet the needs of the individuals we serve.
While we may not have all the answers, we are committed to helping and supporting in every way we can, and will always ensure to follow-up.

Meet Benjamin, our oldest son. Benjamin was diagnosed with Autism at the age of 2. Some may say that is too young to diagnose; however, we believe in early access to supports to help our little ones be as happy, healthy, and safe as possible. We are committed to ensuring families are heard and helping support those who may be going through similar challenges and navigating this overwhelming system of services. APP strives to do the same for your family, as we would for our own.
